RALEIGH COSTA RICA EXPEDITION HEALTH AND WELL-BEING FORM

This section is to find out a bit more about you and how we can ensure you have the best experience of volunteering with Raleigh Costa Rica. In that sense, we will ask you about any health and well-being issues so that we can risk assess your participation on the Expedition programme. Please be as honest as possible with these questions. 

If you fail to disclose relevant information, Raleigh Costa Rica may ask you to leave Expedition early, if it is deemed necessary to do so. Following submission of this form, a member of the Raleigh Costa Rica staff team will be in contact with you if any additional follow up is required. 

Please also note that failure to disclose information to your own insurance company may invalidate your insurance and leave you liable for any costs you incur. Raleigh Costa Rica cannot be held liable for these costs.
